All Plan Premiums are Non-Refundable. Premiums must be paid in full prior to receiving benefits. Membership is for one calendar year beginning January 1st and ending December 31st. Plan discounts are valid for the current calendar year only and expire after December 31st of the benefit year. Membership discounts do not apply towards dental products (i.e. electric toothbrush, bleaching syringes, etc.) There will be a finance charge associated for services provided using Care Credit or other financing companies while covered under this plan. Patient portions are due in full on the day of services.

This is a savings plan, not a dental insurance plan. This plan is only honored at Premier Dentistry and cannot be used in conjunction with any other dental plan. All discounted rates cannot be combined with other promotions being offered at Premier Dentistry. Membership is not for resale. All Plan Premiums are Non-Refundable.
